If you worked in a Catholic school prior to 1980, please send the form to the Catholic Schools Office. We will verify the information and return the form to you. If you worked in a Catholic school after 1980, there are no teacher records at the Catholic Schools Office. Most teachers since 1980 did pay into retirement programs besides Social Security and this information should be available at the school or rectory.
